Robotic Tracheobronchoplasty: A Novel Technique with Y-Shaped Mesh

OBJECTIVE: This video demonstrates approach to a robotic tracheobronchoplasty performed using a novel technique with a single Y-shaped mesh.
METHODS: A 70-year-old female with severe symptomatic tracheobronchomalacia underwent a robotic tracheobronchoplasty. Preoperative dynamic CT scan and dynamic bronchoscopy are shown. Y-shaped mesh construction at the backtable is described, followed by depiction of tracheobronchoplasty using multiple interrupted mattress sutures in rows of 3-4, extending along the trachea to the thoracic inlet, and distally to the secondary carina in both the right and left mainstem bronchi.
RESULTS: Patient discharged home with improvement in symptoms.
CONCLUSIONS: Robotic tracheobronchoplasty using a novel Y-shaped mesh is a viable option that can improve efficiency compared to a classic robotic tracheobronchoplasty with multiple discrete pieces of mesh. This does require careful preoperative planning and precise backtable approach to accommodate different tracheobronchial angles as part of the Y shaped mesh configuration.
