Excision of a Cardiac Paraganglioma

OBJECTIVE: To demonstrate the intraoperative presentation and surgical excision of a cardiac paraganglioma, a rare cardiac tumor.
METHODS: After median sternotomy, the pericardium was opened and the mass was found to overlie the right ventricle. The patient was initiated on cardiopulmonary bypass. Cardioplegia and aortic crossclamping were initiated and the mass was carefully excised from the right ventricular wall adjacent to the outflow tract. The proximal left anterior descending artery was within the resection cavity and that segment was bypassed with a pedicled internal mammary graft.
RESULTS: After weaning from bypass, the heart spontaneously recovered a normal sinus rhythm. The patient was brought to the cardiac intensive care unit in stable condition. He was discharged home on postoperative day six.
CONCLUSIONS: Cardiac paraganglioma is a rare tumor with a characteristic intraoperative presentation, and can be successfully treated with surgical excision.
